Don’t have a referral from your physician? No Problem! Kelsey will provide a 30 minute screening to see of Physical Therapy would be beneficial. We will then contact your provider notifying them of the need for Skilled Therapy!

Self Pay Initial Evaluation. 1 on 1 initial visit where the therapist takes a detailed history and makes an assessment of your limitations and forms a plan of treatment with you!
